Occupational health testing services including DOT physicals, pre-employment physicals, as well as, drug and alcohol testing, that meet the federal requirements for FMCSA, FAA, FTA, FRA, PHMSA, and the USCG. We can offer these services on-site at your facility.
We also perform peer review of chiropractic services to insurance carriers, third-party administrators (TPAs), independent review organizations (IROs) and attorneys.

You should bring a list of all medications/supplements you take and medical records detailing any medical condition you may have. These records should include your doctor's name and address. If you wear eyeglasses and/or hearing aids, bring them because vision and hearing tests will be performed. Also, if you take prescription medication, bring the bottle.
Be sure to get plenty of rest and drink adequate amounts of water prior to your exam.
You must pay at the time of service if you do not have prior approval from an employer that has an account with us. If you do have prior approval from such an employer, then you either pay the pre-negotiated price or your services may be covered and cost you nothing at all.
Because many insurance companies treat these services as preventative health care and do not cover them, Industry Health has a self-pay policy in which you pay us directly at the time of service and then submit the receipt to your employer or insurance company for reimbursement. Owner/operators and independent contractors also have the option of writing-off the cost of these health-related expenses on their taxes.
A certified medical examiner is a doctor of medicine (MD), doctor of osteopathy (DO), or doctor of chiropractic (DC) who has completed additional training to become certified by the Federal Motor Carrier Safety Administration (FMCSA)—a division of the United States Department of Transportation (USDOT)—to perform physical qualification examinations (“DOT physicals”) for commercial drivers.
The certified medical examiners at Industry Health are doctors of chiropractic who have completed the appropriate training and are listed on the National Registry of Certified Medical Examiners (NRCME). Doctors of chiropractic use a conservative approach to healthcare that focuses on neuromusculoskeletal health, ergonomics, and overall well-being.
